    GOSHEN PLUMBING & HEATING CO., INC.: Also known as The Company, agrees to provide you (The Customer), with the necessary labor and maintenence materials to perform a comprehensive maintenence program for your heating and/or cooling equipment.
    Discount: The Company will provide a 10% discount on all repair parts. Emergency labor will be reduced by $2.00 per hour below our prevailing labor rate, as well as, no service call fee.
    PREFERENTIAL SERVICE: The Company will provide emergency service to The Customer before non-agreement customers.
    HOURS OF SERVICE: The service covered by this agreement will be scheduled during The Company's regular working hours, 8:00 a.m. to 5:00 p.m., Monday through Friday, except holidays. After hours or weekend scheduling is available for an additional charge.
    PARTS: The agreement does not include any parts or materials except maintenence materials (lubrication/cleaning supplies) necessary to perform the required maintenance. Standard 1" filters and thermo-couples are included in this agreement. ALL other parts are provided for an additional fee.
    REPAIRS: Any repairs required beyond those provided in this agreement will be brought to The Customer's attention and will be billed on a time and material basis. Payment is due at the time of service. The Company shall not be liable for any loss arising from delay, failure to discover a condition requiring repair or replacement, or as a consequence of any performance under this agreement.
    RECORDS: The Company will maintain a complete record of all maintenance and repairs performed by The Company and will provide The Customer with a written report upon completion of any work.
    TERMS: This agreement is for one year; January to December, annually.
    BENEFITS: There aer many more benefits to joining our service program than those listed above. Some other benifits include: lower utility bills, fewer inconvenient service calls, longer equipment life, peace of mind, improved safety, and being a good citizen through helping conserve our nation's energy supply.
    PLAN REQUIREMENTS: The Company must have serviced or installed the equipment within the year prior to the activation of the agreement. Therefore, you may not skip a year under this agreement. If one year is skipped, there will be a $40.00 reactivation charge to continue with the agreement.
